Submit Your Month 3 Eye Tests
Please upload your Month-3 R (Right Eye), Month-3 L (Left Eye), and Month-3 B (Both Eyes) test results here. Be sure to print each chart on 8.5” × 11” white paper at 100% scale (not “fit to page”) for accurate results.
How to Submit the Month-3 Eye-Test
Step 1: Download & Print the Eye Test Package (If you have not done so already)
Step 2: Complete the Month-3 Eye Test
Read the instructions carefully, then do the Eye Test for:

Right eye (M3-R)

Left eye (M3-L)

Both eyes (M3-B)
Log every line you can read, and record your results in the Vision Log table provided.
⚠️ Note: Without submitting the Month-3 test, you will not be enrolled for the Month-4 program
**Please make sure to TAKE THE EYE TEST WITH YOUR GLASSES OFF. This allows us to test your natural vision, not your corrected one.
