Please upload your Month-1 R (Right Eye), Month-1 L (Left Eye), and Month-1 B (Both Eyes) test results here. Be sure to print each chart on 8.5” × 11” white paper at 100% scale (not “fit to page”) for accurate results.

How to Submit the Month-1 Eye-Test
Step 1: Download & Print the Eye Test Package
Step 2: Complete the Month-1 Eye Test
Read the instructions carefully, then do the Eye Test for:

Right eye (M1-R)

Log every line you can read, and record your results in the Vision Log table provided.
